Application for Jerome Formo Award

 

The Jerome Formo Award is designed to highlight the best of the Upper Midwest Section’s future plastics professionals.  Candidates must be:

•      Member of the Upper Midwest Section in good standing

•      Full-time student

•      Enrolled in a 4-year college program

•      Demonstrate interest in the plastics industry by majoring in a course beneficial to a career in plastics and attendance at SPE events

•      Must be in good academic standing

•      GPA better than a 2.5 on a 4.0 scale

•      Submit a (minimum of) two-page statement containing

o   why you are applying for the scholarship

o   your qualifications, and

o   how your study of plastics will contribute to your immediate and/or long-range career goals.

•      A completed application form

•      Recommendation letters from at least two instructors

 

All submissions received prior to (or having a postmark date by) 5:00 p.m. CST on the "submission date," will be reviewed by the Upper Midwest Section Scholarship Committee.  If a winner is identified they will be notified by e-mail and invited to an SPE event where the recipient will receive a $500 (USD) scholarship award and a certificate.
